Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

Specialty Programs
Our Center is focused on adding specialty clinical programs to better serve patients. In some cases, additional training and education can be provided for staff on specialized equipment and treatment programing that may enhance recovery times. SEE ALL SPECIALTY PROGRAMS

Your loved one deserves much better than this place. The joining bathroom has had urine on the floor and wall for 2 days. All of this was reported to supervisor to no avail.

[review] => After reading a few good reviews, I thought it can't be bad. Smelled clean and was bright. That's the end of anything positive. Unfortunately, my dad was there for 3 weeks before I could get him moved. He came for rehab after a 2 week hospital stay to help build his strength up. He left in worse shape. He had 1 shower the beginning of his stay and not another one until a week and a half in. He smelled awful. My son and I had to keep helping him change clothes because he was filthy every time we visited. He said he would call for help if he had an accident in his bed and at least on one occasion they made him lay there for an hour in his own urine. PT was maybe 1 hour a day then he was sent to his room and confined to his bed or wheelchair. He said they got mad if he tried to get up. They are understaffed and difficult to get answers from. Always shooed me away and said they would come to the room. Tried to call the administrator but was told she was in a meeting. I left a message to return my call, she never did. The furniture in the rooms are antiquated and highly uncomfortable. My dad cut himself multiple times on the bed. Found him one day with dried blood all down his leg and on his sheets (stayed for 2 days). Wounds were barely cared for and bandages left on for days. Patient bathrooms were old and nasty. My dad said many of the staff members were always arguing. I witnessed this myself, I even heard a male staff member yelling at a patient in the cafeteria, then later complaining to a nurse how it wasn't his job, etc. She yelled back at him. All outside my dad's room. They are noisy and have no consideration for quiet time in the evenings when people try to sleep. My dad was miserable and kept begging me to get him out. When I came to pick him up, it smelled heavy of bleach and many of the staff were wearing masks. Walked in and my dad had "a head cold". I asked multiple staff if COVID was going around. They completely ignored my question or referred me to their web page. Nothing was posted on the doors or website as I could find. Brought my dad to his new assisted living where he tested positive for COVID two days later, which he passed along to myself and my son. I complained to the social worker a week before he was discharged and she claimed she was filing a grievance report. Never got a follow-up if that even occurred. Had one meeting two weeks in to even get an update on his care, which was minimal. One evening they forgot to bring his dinner. I had to hunt it down for him. Thankfully I was there that evening. I could go on and on. Please do NOT trust this place to care for your loved one.

[review] => Our friend and neighbor was there for rehab after a stroke. The staff was very caring and attentive. They also allowed his friends to visit often and in large numbers to bring a little bit of home to him while he was recovering. Some have rated FWRC low because of the response time to the call button, but these people have unrealistic expectations. They do very well with the staff they have. They really care for their patients and it shows.

My mother was sent to Ft Walton Rehab Center by Emerald Coast Hospice and her caseworker. She was only there for a Respite stay of 5 days starting on 1/20/2023. The first night she was there, she fell and the staff there called 911 instead of calling Emerald Coast Hospice first as well as her caseworker. My mother also wore the same pants and shirt for 2 days, I had put clean clothes in the top and middle drawers in the nightstand next to bed 1/22/2023. On 1/23/2023 the doctor from the facility came in to examine my mother and when I commented that Mom was there for Respite only and she was to stay there for 5 days only and he replied "Ohhhhh", like he didn't know she was there only for Respite. I got the impression that he thought she was going to be a permanent patient. After all said and done, I requested my mom be discharged on 1/24/2023 instead of 1/25/2023 with the assistance of her caseworker at Emerald Coast Hospice. I got her back home on 1/24/23 and Mom had to use the restroom and when I helped her to pull her Depends off I noticed she had a really bad diaper rash, which she didn't have before I took her to Ft Walton Rehab Center on 1/20/2023 !! I'm not happy with the way my mother was treated at this facility Ft Walton Rehab Center. Unsatisfied son.

[review] => My uncle stayed here after being hospitalized for sepsis and they took such great care of him and got him back up on his feet. Even after being released from therapy they kept him until he was finished with his IV antibiotics. Everyone here was always so friendly and helpful. My uncle was very happy here and said he wouldn't mind staying as a long term resident if needed!

[review] => first off my husband was in this rehab for about three weeks then they let him go while being sick. Someone at the rehab was sick and gave it to him! He's 83 and this is not ok!! on top of that they sent him home while majorly loaded!! not sure if it was the flexeril they had him on twice a day for a week or something else but he was extremely out of it and my husband does not take meds to cause him to be loaded! Thirdly the day before his release I almost stepped on a syringe with a bare needle at the entrance of his bathroom. Seriously!!??? This rehab leaves a lot to be desired in their competence! I also believe they let him go early because they didn't want liability! The staff was kind and congenial and they did do their job as well as can be expected. None the less this rehab leaves a lot to be desired!

This facility and staff have EXCEEDED my expectations in every way.
I'm 57 and have been here a month recovering from a stroke. They go above and beyond to ensure medical, mental and physical needs are always their number one priority and truly listen to the patient's and the family members to better help with rehab and level of comfort. They become an extension of the family. They have provided such a positive impact on my life I can truly only say THANK YOU for treating me with such respect and helping me regain my independence.

[review] => This facility has been amazing to my entire family. My father is a patient there currently and not only are the nurses, CNA's and therapist great, the Administrator Ashley is phenomenal! Ashley has been a true God-sent and makes time to engage her patients and their families. Ashley has a heart warming approach and has a gift for making sure patients and their families know that they are also her top priority while still performing her job meticulously. Also Ms. Bernice that answers the phones is beyond Amazing! Our whole family had a very rough time when my father was admitted and Ms. Bernice was very helpful and empathetic. Our family was truly blessed that my father was able to conduct his rehabilitation at FWB Rehabilitation Center.

[review] => My father caught Covid after 3 weeks as a patient. He was transferred to FWBMC by EMS and passed away 4 days later. Patients are isolated from visitors for 14 days upon entering and if you can get visitation you are still separated from your loved one. So the question is “how does the virus get in the faculty?
Well, not all the caregivers have been vaccinated and caregivers are not screened when reporting for work!
IMHO if a caregiver is not willing to be vaccinated against a deadly, Highly communicable virus, maybe they shouldn’t be interacting with High Risk patients!
We are considering all options.

Even threatened me Medicaid wouldn't pay if I left AMA. I am on MEDICARE. I left AMA less than 18 hours after arrival. Looks nothing like the photos they have up.

[review] => I have a close friend that I am caring for going into this rehab as I type this in going to stay in the 3 star range until I can give be swayed one way or the other. I will be looking at the concerns mentioned in the negative reviews for call lights not being answered (huge concern for me) and UTI coming from being catheterized as my friend can not get up and down on his own I'm concerned if this is a plan of treatment. I will not stand for pee smells or roaming "zombies". I agree this is no way to improve the mindset of a patient in for rehab. I will be happy to report on all the great points I read of friendly staff and care and compassion by rehab staff and nurses. It is true we read reviews more than ever these days because it's a hard decision trusting your loved ones with someone else to care for them. So God Bless us All. Family, Friends & Caregivers Alike. [review] => I have been in long-term care facility ministry for several years. I have never seen a better facility than Ft. Walton Rehabilitation Center, it is clean and the staff and residents enjoy each other’s company. I have seen staff sing to residents, pray with residents, and genuinely care for our precious residents and each other. The corporate management team is just a phone call away and care about our staff and residents. [review] => My Mother was at this facility for two weeks. Its a good thing she had family to take care of her because there is not enough staff, it took more than 30 minutes for help to go to restroom and to top it off there were, on two occasions, no hot water for bathing for more than three days. We had to take our mother home to bathe! Food was always cold and was served without a drink. Meds were not given correctly as the drs ordered changes and nurses didn't have the orders for days later. Therapist were great but sad that they had to work under those conditions. Several of these conditions are certainly agains health regulations and should be documented. In conclusion, who should be contacted to report this facility and provide a competent facility for our ailing senior citizens that need medical attention.

In a brief response to your review and for others who may be reading this, we had an unfortunate water heater event which involved a part that took a week to procure. We certainly responded as appropriately as possible during what we considered an emergency type situation and were able to provide great service to our other patients, albeit not without some similar concerns. We also worked tirelessly with officials as we progressed through completion of our repair.
We always encourage family support at any and all levels including contacting facility administration. It is the primary focus of my job. We are certainly not perfect but maintain a very high standard because of our service response.
I hope you will consider reaching out to me so we can completely understand any shortcomings you experienced.

[review] => My mom had to come here from the hospital and her care was great the staff are always kind and caring and listening to our needs. I felt like the business office and the administrator were very helpful with any difficulties that came up while we were there. My mom had food allergies and the kitchen tried extremely hard to accommodate her. Overall like any place my mom has stayed in the past there are always things that can be improved by on, what I found to be the best thing is that they were always willing to work on it instead of trying to cover everything up like we saw at the hospital. Thank you to rehab, nursing and activities for keeping my mom motivated. Your caring shows. We had a very positive experience!

[review] => The PT department was wonderful and everyone was friendly but patient care had a lot to be desired. They are understaffed and can't keep up with the patient's needs. It has taken up to an hour more than once to have someone come in when you use the call light. Most CNA's were wonderful but you always have those few that doesn't put the patient first.
My mother chose Fort Walton Rehab because her doctor makes rounds there. She went there for rehab due to a knee replacement and ended up falling one week after having her knee replacement, (her fault) and broke her left humerus bone. She didn't have surgery immediately on her arm and was in excruciating pain with her arm and some of the CNA's didn't even take that into account. She had to have a catheter foley put in because she was totally in able to get up on her on and go to the bathroom. I repeated over and over I didn't think her urine looked good and they just kept saying she needed to drink more water. I took her to Scared Heart ER because she was in so much pain and they did surgery later that afternoon on her arm. She tested positive for a UTI and E.coli. Needless to say I did not take her back to that rehab and would never recommend that place to anyone. I put her in rehab in Destin and she is doing so well and they are so attentive to her needs, which is what a patient needs during times like these. I feel for the nursing home patients who doesn't have someone to be there voice. It really saddens my heart that they can't get the extra attention they need due to being understaffed.

[review] => My mother died as a result of and untreated UTI that turned into Urosepsis while living at this facility. One month prior to being hospitalized because I found her unresponsive in front of the nursing station, she had a bruise that covered her entire left forearm with a gash that no-one on staff could explain how it happened. They shrank her clothes, put her in clothes that belonged to someone else and that were much to large for her even when she had a closet full of her own clothes. I made repeated complaints about the laundry, dirty room and filthy wheelchair. All of which I have photos.

It is a place that cares about the patients and is family friendly, it has one of the best wound care programs in North West Florida if not the BEST! I came here for wound care 4 months ago and all of my needs is meet by the staff here from the business office all the way to the C.N.As, they have a wound care Doctor and facility Doctor that visit weekly and the Nurses can call them at anytime if needed, I have been in an out of Rehabilitation / Nursing facilities for almost 11 years from Thallahass all the way to Pensacola and I most say this is the BEST one yet!!! So if you are concern about your love one's and know that they are cared for Fort Walton Beach Rehab facility is the way to go FOR THEM believe me I know cause I'm still here.

[review] => Ted gilstrap, I just spent 13 days at Westwood and it was the most satisfying stay I have ever experienced.
The staff was extremely professional, caring, attentive and totally friendly.
They created a warm and totally comfortable atmosphere.
Their therapist were firm but gentle, persistent but thoughtful and as they put me thru the various exercise they were very aware of my original short comings, my weaknesses and were fully in control of my body thru out each exercises.
I have never feel so confident as to their control and protectionism during ever work out.
If there were a 6+ in the rating system I would gladly do so.
Super people, super facility and super experience.

[review] => This is not a 5 star rehab, the staff is friendly but their plan of action is to keep you in there as long as your insurance allows. Their wound care sucks. Its also a nursing home with zombies roming the halls, urine smell, they should not mix rehab and nursing home. Brings down the hope of rehab patients to see those people in nursing situation. Wont be back ever.

[review] => My father is currently residing at the FWB Rehab. for short term therapy after a horrible car accident. My family is very involved with his care and there is someone there visiting him everyday. The staff are very attentive and always address our concerns and needs right away. They schedule a care meeting with me and the PT, OT, and Speech Therapist are always working with us to ensure that we are all on the same page regarding his short and long term goals. I could not be more pleased with his care. Nurse Shawn and Speech therapist Lindsey are the best and have really made a connection with both my father and our family.
