Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

[review] => I moved my dad from another local facility to Fort Walton Rehabilitation Center in April of 2024. He was so relaxed there and had his favorite CNA's and nurses. The care he was given by all staff members, was fantastic. He was treated like a member of the family. He liked being able to sit outside in the front or in the court yard. Communication by the staff to me was excellent. I highly recommend taking your loved one here for long term care.

[review] => My 92 year old dad was admitted to Ft. Walton Rehab after a short hospital stay. This review is based on my daily visits to see my dad while in the facility.
The Rehab wing always had patient and upbeat workers. My dad loved each of them. To say they were patient with the patients doesn't begin to truly convey how patient they were. My dad wasn't always the easiest patient. Eventually, he hit a point of no progression. We made the decision to move him to the extended care wing, (a.k.a. nursing home). He loved the nurses and they were so caring. I was able to always get time with the nurses to discuss my dad's disposition and general health condition for the day. If he didn't feel like feeding himself, they would sit with him and feed him. They worked with him to make sure he was out of bed for several hours a day to keep him moving somewhat and having some social interaction. I joked that they were "holding court" as there were numerous patients in their wheelchairs encircled around the nurses station. They were interacting with the nurses and it was not unusual for them to be giving them snacks of shakes or pudding while they were "holding court". There was always residents going up and down the halls in their wheelchairs. Some of them visiting with others while they did so.
My dad was always clean. His clothes and dressings were always clean. His bed was clean. The room and facility was clean. The beds were good and operational. They didn't look old and rusted like the previous rehab he was in during a different health event. Even his roommate, who had apparently had a massive stroke, appeared to be clean and cared for. It was not unusual for the nurses to come in and talk to the roommate who really couldn't communicate. I was happy to see that they didn't leave him alone just because he couldn't move and communicate. I can attest that the roommate was always clean as well. Never was my dad or his roommate left to sit in their dirty underwear or clothes. The nurses were always friendly and seemed happy to be there. This was a stark contrast to the previous rehab he had been in earlier in the year. They were quick to call when he had the slightest sore developing or even a change in attitude with him. They kept in close contact with doctors concerning my dad's condition. I always knew he was being well cared for and that brought us peace.
The administration was helpful and generally friendly. They knew the residents and greeted them whenever they were in the halls. They helped us with working on financial arrangements, giving us paths to reach out to for financial help with Medicaid and insurance so we were able to afford their services.
When my dad's condition declined, we were called immediately. They kept us updated and when we were there, they made room for us in the room. Never once did I feel like we were in the way or a hinderance for being there. There was a CNA that sat outside his room the last hour we were there when he was declining, giving us some space but being immediately available if we needed her. Even near the end, they made sure he was clean. At the end (the last 10-15 mins), a nurse came and was with us explaining what to expect and helping us with the emotional experience of my dad's passing. They gave us time and space with him afterwards. It was a very peaceful experience. Far different than what I had envisioned being with my dad when he passed.
Needless to say, my family was so pleased with Ft. Walton Rehab and glad he was with people who loved and cared for him and he loved them.

[review] => My mother celebrated her 95th birthday on Sunday. I want to recognize the amazing work of the entire staff at Fort Walton Beach Rehabilitation Center. My mother has been in and out of the rehab center since 2019 and I can say, without hesitation, she would not be alive today without the care she has continuously received. It is not only the care, but the compassion within every department evidenced in so many interactive situations.
With our history of stays, it is always comforting to see familiar faces, smiles and welcoming comments from both the staff and people that stay in the facility long term. They are happy.
I understand, of course, the situations might not be ideal for any patient, but life continues and how wonderful to be well taken care of in the most difficult of times.
Bravo for jobs well done! Thank you for taking care of my mom and so many others.

We had minimal time to figure out a health plan and NO help from her previous rehab facility. In our moment of need, FWB rehab was there. We could not have managed alone. I am so thankful to Jayme and Shatilla in Admissions. They are absolute rockstars. Not only did they step in and take over, but they eased our minds and made my mother feel valued, respected, and heard from the moment she arrived. We are just a few days in and her progress has surpassed what she had achieved in 2 months at Pruitt Health in Panama City. What started out as a nightmare has become an absolute dream. We hate how it happened, but are so thankful it did. All love for this facility. Thank you for your commitment to the people you serve.

[review] => After reading a few good reviews, I thought it can't be bad. Smelled clean and was bright. That's the end of anything positive. Unfortunately, my dad was there for 3 weeks before I could get him moved. He came for rehab after a 2 week hospital stay to help build his strength up. He left in worse shape. He had 1 shower the beginning of his stay and not another one until a week and a half in. He smelled awful. My son and I had to keep helping him change clothes because he was filthy every time we visited. He said he would call for help if he had an accident in his bed and at least on one occasion they made him lay there for an hour in his own urine. PT was maybe 1 hour a day then he was sent to his room and confined to his bed or wheelchair. He said they got mad if he tried to get up. They are understaffed and difficult to get answers from. Always shooed me away and said they would come to the room. Tried to call the administrator but was told she was in a meeting. I left a message to return my call, she never did. The furniture in the rooms are antiquated and highly uncomfortable. My dad cut himself multiple times on the bed. Found him one day with dried blood all down his leg and on his sheets (stayed for 2 days). Wounds were barely cared for and bandages left on for days. Patient bathrooms were old and nasty. My dad said many of the staff members were always arguing. I witnessed this myself, I even heard a male staff member yelling at a patient in the cafeteria, then later complaining to a nurse how it wasn't his job, etc. She yelled back at him. All outside my dad's room. They are noisy and have no consideration for quiet time in the evenings when people try to sleep. My dad was miserable and kept begging me to get him out. When I came to pick him up, it smelled heavy of bleach and many of the staff were wearing masks. Walked in and my dad had "a head cold". I asked multiple staff if COVID was going around. They completely ignored my question or referred me to their web page. Nothing was posted on the doors or website as I could find. Brought my dad to his new assisted living where he tested positive for COVID two days later, which he passed along to myself and my son. I complained to the social worker a week before he was discharged and she claimed she was filing a grievance report. Never got a follow-up if that even occurred. Had one meeting two weeks in to even get an update on his care, which was minimal. One evening they forgot to bring his dinner. I had to hunt it down for him. Thankfully I was there that evening. I could go on and on. Please do NOT trust this place to care for your loved one.

My mother was sent to Ft Walton Rehab Center by Emerald Coast Hospice and her caseworker. She was only there for a Respite stay of 5 days starting on 1/20/2023. The first night she was there, she fell and the staff there called 911 instead of calling Emerald Coast Hospice first as well as her caseworker. My mother also wore the same pants and shirt for 2 days, I had put clean clothes in the top and middle drawers in the nightstand next to bed 1/22/2023. On 1/23/2023 the doctor from the facility came in to examine my mother and when I commented that Mom was there for Respite only and she was to stay there for 5 days only and he replied "Ohhhhh", like he didn't know she was there only for Respite. I got the impression that he thought she was going to be a permanent patient. After all said and done, I requested my mom be discharged on 1/24/2023 instead of 1/25/2023 with the assistance of her caseworker at Emerald Coast Hospice. I got her back home on 1/24/23 and Mom had to use the restroom and when I helped her to pull her Depends off I noticed she had a really bad diaper rash, which she didn't have before I took her to Ft Walton Rehab Center on 1/20/2023 !! I'm not happy with the way my mother was treated at this facility Ft Walton Rehab Center. Unsatisfied son.

He's 83 and this is not ok!! on top of that they sent him home while majorly loaded!! not sure if it was the flexeril they had him on twice a day for a week or something else but he was extremely out of it and my husband does not take meds to cause him to be loaded! Thirdly the day before his release I almost stepped on a syringe with a bare needle at the entrance of his bathroom. Seriously!!??? This rehab leaves a lot to be desired in their competence! I also believe they let him go early because they didn't want liability! The staff was kind and congenial and they did do their job as well as can be expected. None the less this rehab leaves a lot to be desired!

Ashley has been a true God-sent and makes time to engage her patients and their families. Ashley has a heart warming approach and has a gift for making sure patients and their families know that they are also her top priority while still performing her job meticulously. Also Ms. Bernice that answers the phones is beyond Amazing! Our whole family had a very rough time when my father was admitted and Ms. Bernice was very helpful and empathetic. [review] => My husband stayed in Fort Walton Rehabilitation Center until he died. I was very happy with them. Everybody was very busy taking care of their clients. It was a large community, so there was plenty of space for everybody. The main thing is that they were very, very diligent about COVID. Your temperature has to be taken before you can come in, and they'll let in two people at a time. I was appreciative of that. It was spacious enough, and they had different hallways for the people. It was an older building, but I was completely satisfied with it. They had bingo and different little activities, but my husband was so bedridden that he didn't get to go to those activities. This facility was close to where we live, and so it just worked out 100%. The staff members were very friendly. They always did what they were supposed to do every day, they checked on him, they changed him, they made sure his bed was good, and they took care of him.

Patients are isolated from visitors for 14 days upon entering and if you can get visitation you are still separated from your loved one. So the question is “how does the virus get in the faculty?

IMHO if a caregiver is not willing to be vaccinated against a deadly, Highly communicable virus, maybe they shouldn’t be interacting with High Risk patients!

I will be looking at the concerns mentioned in the negative reviews for call lights not being answered (huge concern for me) and UTI coming from being catheterized as my friend can not get up and down on his own I'm concerned if this is a plan of treatment. I will not stand for pee smells or roaming "zombies". I agree this is no way to improve the mindset of a patient in for rehab. I will be happy to report on all the great points I read of friendly staff and care and compassion by rehab staff and nurses. Its a good thing she had family to take care of her because there is not enough staff, it took more than 30 minutes for help to go to restroom and to top it off there were, on two occasions, no hot water for bathing for more than three days. Several of these conditions are certainly agains health regulations and should be documented. It has taken up to an hour more than once to have someone come in when you use the call light. Most CNA's were wonderful but you always have those few that doesn't put the patient first.
My mother chose Fort Walton Rehab because her doctor makes rounds there. She went there for rehab due to a knee replacement and ended up falling one week after having her knee replacement, (her fault) and broke her left humerus bone. She didn't have surgery immediately on her arm and was in excruciating pain with her arm and some of the CNA's didn't even take that into account. She had to have a catheter foley put in because she was totally in able to get up on her on and go to the bathroom. I repeated over and over I didn't think her urine looked good and they just kept saying she needed to drink more water. I took her to Scared Heart ER because she was in so much pain and they did surgery later that afternoon on her arm. She tested positive for a UTI and E.coli. Needless to say I did not take her back to that rehab and would never recommend that place to anyone. I put her in rehab in Destin and she is doing so well and they are so attentive to her needs, which is what a patient needs during times like these. I feel for the nursing home patients who doesn't have someone to be there voice. It really saddens my heart that they can't get the extra attention they need due to being understaffed.

[review] => My experience with Fort Walton Rehabilitation Center so far has been very positive. The staff seems quite professional. Everything is just spic and span and great. My father said the food is very good, and he is happy with that. The common areas outside are great and includes a wonderful gazebo in the back. This will be my father's second day, so it's kind of early in the game, but as of right now, I would certainly recommend them.

[review] => I visited this facility and the staff wasn't helpful at all.I was looking for someone and they didn't know they where,told me just to look around.The nurses were at the nurses station standing around talking and on cell phones.I wouldn't put anyone in there if it was free.I've talked with other people who have had plenty of complaints about this place.

The staff is very attentive, and it is sufficient staff for the community that resides in there.They have a very good operation, it's very clean, and the food is good. I haven't tried the food personally, but judging from my mother-in-law, she eats it up pretty much on a regular basis. I am impressed with the attentiveness of the staff and the cleanliness of Fort Walton Rehabilitation Center.

[review] => Parthenon Healthcare Of Fort Walton is a very nice facility. It is very clean and one does not get the all too familiar "nursing home" smell when entering the facility. I always see the cleaning staff cleaning the halls and rooms when I'm there. The staff has always been very nice, helpful, and accommodating to us whenever we have questions. I've noticed that they are also very nice and attentive to the patients as well. The patients always look clean and look well cared for.On the downside, the patients often complain about the food. The food looks good, but they say it does not taste good. Patients that are well enough to go to the dining room, get a hot fresh meal. However, the bedridden patients often complain that the food is cold by the time it gets to them. I would highly recommend the company get better food carts and/or trays to keep the food hot, so everyone can enjoy a hot meal.

Being apart of many outreach programs in FWB, I’ve visited this facility many times. The staff is knowledgeable, friendly, and genuinely cares about their patients recovery. The facility is clean, modern, and well-equipped, creating a comfortable and welcoming environment. I always feel warm welcome anytime I step into the building.

They took the time to listen to concerns and goals, creating a plan that will help make significant progress during your time there.
